His grandmother reared him in poverty in the Smoky Mountains after his mother abandoned him as a child, and he attended the Governor Morehead School for the Blind in Raleigh until he was five years old. He first fell in love with music as a child, especially late-night radio broadcasts of rhythm and blues, gospel, and country music.

His teachers noticed his musical potential, and when he was seven, he started taking official classical music lessons at Governor Morehead State University and picked up numerous instruments.

Unfortunately, he received a smack from a school parent, losing the minimal eyesight in his left eye. Milsap started listening to rock and roll music and started the rock group Apparitions with other high school classmates.

To become a lawyer, he was given a full scholarship at Young Harris College in Young Harris, Georgia, where he briefly studied. He was a part of the Dimensions, a well-known neighborhood R&B group that played in the Atlanta region at the time. He dropped out of college and declined a scholarship to go to law school to focus solely on his musical career.

In the 1960s, he moved to Memphis, Tennessee, where he worked with producer Chips Moman and established himself as a weekly fixture at Memphis nightclub T.J’s.

The same year he reached the bottom echelons of the pop charts with the track “Loving You Is A Natural Thing,” he also recorded and published his debut album. After an accidental encounter with music icon Charley Pride, who was seeing Milsap sing at the Los Angeles nightclub Whiskey, he relocated to Nashville in December 1972.
