Katie Wilkins tragically died on July 31, 2020, as a result of a catastrophic brain bleed. Her life was taken by the negligence of Liverpool’s Alder Hey Children’s Hospital and Warrington Hospital. After nearly three years, a government minister has addressed the medical deficiencies that led to the tragic death of the teenager with a “big heart.”
On Monday, February 20, coroner Katy Ainge issued a PFD report to the health and social care department. The Warrington teen was seeking treatment after being diagnosed with a rare form of leukemia. Katie Wilkins’ death has been attributed in part to a national shortage of hematology specialists. The Cheshire girl died on July 31, 2020, at Alder Hey Children’s Hospital. Katie visited Warrington Hospital several times before passing out at home on October 26, 2020. She was taken back to the hospital for more blood tests.
The adolescent was taken to Alder Hey hospital and diagnosed with coagulopathy, a bleeding disorder, and acute promyelocytic leukemia (APML). Katie’s condition deteriorated, and she suffered a severe intracerebral hemorrhage on July 29. As a result, the Cheshire girl required emergency surgery but never recovered. Medical personnel declared her dead two days later, on July 31.
A government minister released a report on preventing future deaths
Concerns about Katie’s care at Warrington and Alder Hey hospitals were raised during an inquest last May. Kate Ainge, an assistant coroner for Liverpool and the Wirral, determined that “neglect” played a role in her death. Ms. Ainge responded by submitting a PFD (Prevention of Future Deaths) report to the Department of Health and Social Care.
Katy Ainge stated in a report released on Monday, 20 February 2023, that “the inquest brought to light persistent concern that Alder Hey Trust’s primary consultants for the care of APML patients will continue to be Cancer specialists.” According to the report, “the risk of significant bleeding caused by severe and related coagulopathy poses the most significant fatality risk in such patients.”
A consultant hematologist who testified as an expert witness at the inquest and was supported by the trust’s top hematologist agreed that coagulopathy management should be overseen by a hematologist to prevent further deaths from this problem.
Furthermore, the report blamed the incident on a national shortage of hematologists, which left Alder Hey trust without resources to address the issue.

Katie Wilkins was from Warrington, Cheshire, England. The 14-year-old was a Bridgewater High School graduate. Similarly, she enjoyed horseback riding. Her family was concerned that a similar incident would occur to someone else in the future. Katie Wilkins’ mother, Jeanette Whitfield, and father, Jonathan Wilkins, spoke to the ECHO shortly after the conclusion of the inquest last May.
The bereaved mother described her daughter as a fun and sassy young lady who enjoyed makeup. “She was bright, caring, and upbeat.” She had a big heart and shone brightly. Mrs. Wilkins added, “Her laugh was so distinctive that I could hear it from a mile away.”
The inquest, according to the family, was extremely difficult to hear. They were grateful, however, to the coroner and witnesses for assisting them in determining the truth about what happened to their precious daughter.
